MORE INFORMATION - www.DEATH CERTIFICATESUSA.com

A Death Certificate is an official and legal document that is issued and filed after the death of an individual. The procedure can vary from state to state, but after an individual has been officially pronounced dead, usually by the hospital, or by a doctor, they are transferred to the morgue. There, an autopsy may or may not be performed. If an autopsy is carried out then an autopsy report is filed along with a coroner's report, sting cause, place and time of death, and this record is held at the Coroner's Office. After this an official Death certificate is issued, and this is held on file at the County Office of Vital Statistics (or Vital records).

The document is an official and legal document that records the individual's passing, and it contains a wealth of pertinent information. The record usually contains all or some of the following:

  • Place of Death, State City and County

  • Place of Death (hospital, hospice etc.)

  • Date and Time  of Death

  • Cause of Death

  • Full Name of the Deceased

  • Age, Date and Place of Birth

  • Usual Residence of the Deceased

  • Sex

  • Race

  • Single, Married, Divorced, Widowed

  • Occupation

  • Name and Birth Place of Father

  • Maiden Name and Birth Place of Mother

  • Name and Signature of Person Attesting to this Information

  • Armed Forces Status

  • Medical Certification of the Certifying Doctor or Coroner

  • Type and Place of Burial or Cremation

  • Signatures of Witnesses

  • Names and signatures of Undertakers Morticians, Embalmers

Note: Death certificates issued for soldiers killed in wartime may contain a much reduced set of information. Little more than the date and place of death may be noted, along with an authorizing signature and cause of death ("died of wounds"). The certificate to the right is for a Private George Albert Broome (440955) of the 5th Battalion of the Canadian Expeditionary Force, killed in action on November 7th, 1917. No other information is given, and the certificate was not issued until 1919.
