Death Certificate
is an official and legal document that is issued and filed after the
death of an individual. The procedure can vary from state to state, but
after an individual has been officially pronounced dead, usually by the
hospital, or by a doctor, they are transferred to the morgue. There, an
autopsy may or may not be performed. If an autopsy is carried out then
an autopsy report is filed along with a coroner's report, sting cause,
place and time of death, and this record is held at the Coroner's
Office. After this an official Death certificate is issued, and this is
held on file at the County Office of Vital Statistics (or Vital
records). |

| Note:
Death
certificates issued for soldiers killed in wartime may contain a much
reduced set of information. Little more than the date and place of death
may be noted, along with an authorizing signature and cause of death
("died of wounds"). The certificate to the right is for a
Private George Albert Broome (440955) of the 5th Battalion of the
Canadian Expeditionary Force, killed in action on November 7th, 1917. No
other information is given, and the certificate was not issued until
1919. |
